The Maternal Infant Health Program connects Medicaid-eligible pregnant people and infants with a nurse and social worker who visit you, coordinate care, and help you reach healthy goals — free as a Medicaid benefit.

How it works
A clear path from first call to ongoing support

Enroll or get referred
Ask your doctor, Medicaid health plan, or contact us directly if you are pregnant or have an infant and are on Medicaid.

Risk assessment
We learn what matters most for your pregnancy or baby and build a plan around your goals.

Nurse & social worker visits
Home, office, or telehealth visits with a caring team who coordinate with your medical providers.

Help with safe sleep, feeding, development, transportation, and community referrals when you need them.
